Strategy Fight Team Oldham

Acknowledgement of risk and waiver of liability.

Please read this waiver and complete the form at the bottom, you will not be allowed to train unless ou have signed this waiver.

1. Assumption of Risk

I understand that participating in Brazilian Jiu-Jitsu (BJJ), grappling, and physical conditioning activities at Strategy Fight Team Oldham involves inherent risks. These risks include, but are not limited to, physical contact, falls, joint manipulation, and the possibility of injury, including serious injury or death. I voluntarily choose to engage in these activities, fully aware of the risks involved. I accept full responsibility for my health, safety, and any injuries or damages that may occur during training, sparring, or competition.

2. Release of Liability

I hereby release and hold harmless Strategy Fight Team Oldham, its owners, coaches, instructors, employees, volunteers, and any other participants from any liability, claims, demands, actions, or rights of action that are related to or arise from my participation in gym activities. This waiver applies to all claims, including those arising from ordinary negligence but not gross negligence or intentional misconduct.

3. Medical Treatment

In the event of injury or medical emergency, I authorise Strategy Fight Team Oldham staff to provide or seek medical assistance on my behalf. I understand that I am responsible for any medical costs incurred.

4. Participant Responsibilities

  • I agree to follow all gym rules, regulations, and instructions provided by coaches and staff.
  • I confirm that I am in good physical health and have no medical conditions that would prevent safe participation in gym activities.
  • I will immediately inform staff of any injuries, illnesses, or conditions that arise during training.

5. Photographic and Video Release

I grant Strategy Fight Team Oldham the right to use photos or videos taken during classes or events for promotional, educational, and social media purposes. If I do not wish to be photographed, I will notify staff in advance.

6. Minors (For Participants Under 18 Years Old)

I, the undersigned parent/guardian, consent to my child’s participation in activities at Strategy Fight Team Oldham. I agree to the terms outlined in this waiver and assume all risks on behalf of my child.
